What to Bring To Your Immigration Physical
In order to ensure your appointment goes smoothly, please bring the following:
- Pre-completed I-693 Form (Report of Medical Examination and Vaccination Record)
- Valid driver’s license or passport
- Vaccination or immunization records
Following the completion of your examination, Dr. Syed will complete the remaining sections of the 1-693 form and seal it in an envelope for your submission to USCIS. Additionally, a copy of the form will be furnished to you for your personal records. It is important to be aware that USCIS will not accept the envelope if it shows any signs of being opened.
